The primary focus of the role is to receive, assist, and direct patients (care navigation) in accessing the appropriate service or healthcare professional in a courteous, efficient, and effective way.
Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
Receive and make telephone calls as required and take messages, ensuring accuracy of detail and prompt appropriate delivery.
Process and complete medical and admin triage forms.
Action repeat prescription requests.
Ensure that all new patients are registered onto the computer system promptly and accurately, including Temporary Residents.
Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone
Administration
To have a thorough knowledge of all practice procedures.
- To work in accordance of written protocols.
Reception
Receiving patients consulting with members of practice team
- Handing completed repeat prescriptions to patient and checking names and address.
- Be able to cover all reception position as necessary
Appointments
Process medical and admin triage forms which are requested on a daily basis.
- Deal with home visits requests
Registrations of new patients computer data entry and medical records.
- Process patients change of address computer data and medical records (have knowledge of practice area).
- Process repeat prescription request in accordance with practice guidelines.
Westfield Road Surgery is a 5 Doctor Practice caring for 8967 patients. We are located in Bletchley Milton Keynes. We have 4 Practice Nurses and 2 Health Care Assistants. We have an experienced management team to lead us.
We are one of the 4 GP practices which form the Southwest Primary Care Network.
Qualifications
Essential: - Previous experience working as a Care Navigator is essential and have a knowledge of using System One
Desirable: - The successful candidate must be a team player and flexible to cover sickness and annual leave.
