Care Navigator

• Act as the first point of contact for patients, carers, and healthcare professionals.
• Provide a friendly, professional, and welcoming service to all patients.
• Handle telephone and online enquiries appropriately and efficiently.
• Assist patients with appointments, queries, and signposting to appropriate services.
• Communicate effectively with members of the practice team and external healthcare professionals.
• Maintain confidentiality and follow practice policies and procedures.
• Work flexibly and collaboratively as part of the wider practice team.

• Making and managing patient appointments in accordance with practice procedures.
• Communicating test and investigation results to patients, following appropriate practice protocols and confidentiality requirements.
• Arranging ambulance and patient transport services where required.
• Receiving and appropriately processing requests for home visits.
• Contacting other healthcare providers and external services on behalf of patients and the practice team.
• Responding promptly to the needs and requests of GPs, Practice Nurses, and other clinical staff during surgeries.
• Accurately entering, updating, and retrieving patient information using the practice computer system.
• Maintaining accurate and confidential patient records at all times.
• Following all practice policies and procedures, including health and safety, confidentiality, and infection prevention and control requirements.
• Providing a friendly, professional, and compassionate service to patients, carers, and healthcare professionals.
• Undertaking any other reasonable duties appropriate to the role and as required by the Practice Manager or senior members of the practice team.
