The Care Coordinator
responsibilities include but are not limited to the following:
1.
Support
our network of four GP practices in achieving clinical quality and population
health management targets, such as chronic disease management, mental health
care, long-term condition reviews, and preventative screening programs. This
includes coordinating patient follow-up appointments, providing information and
guidance by telephone, and distributing relevant health resources, all via our clinical
health records system: EMIS.
2.Plan,
organise and facilitate educational group sessions for targeted patient
populations, helping to improve health literacy, encourage self-management, and
promote proactive wellbeing. Topics include weight management, menopause, men's
health and wellbeing, and pre-diabetes.
3.Build
and maintain strong relationships with social care providers, community organisations,
voluntary sector partners, and local services to strengthen neighbourhood
healthcare initiatives and support the delivery of patient wellbeing events.
Conduct
proactive telephone assessments with frequent healthcare service users and
individuals absent from work due to illness or disability, identifying barriers
to recovery and connecting patients with appropriate health, wellbeing, and
community support services.
4.Support
the delivery of COVID-19 vaccination and Point of Care Testing programmes
through stock and inventory management, including receiving deliveries,
maintaining accurate records, and assisting practice managers with operational

reception and administrative support, including welcoming and checking in
patients attending ARRS services such as Physiotherapy, Dietician, Social
Prescribing, and Clinical Pharmacy. Respond to patient enquiries and coordinate
follow-up appointments to ensure a positive patient experience and continuity
of care.
Havering Marshalls PCN consists of four GP practices in Romford,
totaling approximately 50,000 patients
North Street Medical Care, Romford
The New Medical Centre, Heath Park
Western Road Medical Centre, Romford
St. Edwards Medical Centre, Romford
Through the Additional Role Reimbursement
Scheme (ARRS) the PCN has expanded traditional GP services by employing individuals
to undertake various roles in order to provide more proactive, personalised and
integrated care for patients registered at the four practices. Our ARRS team
consists of Care Coordinators, Physiotherapists, Dietitian, Pharmacists,
Pharmacy Technicians and Social Prescribers, as well as our leadership team
which includes two Clinical Directors, PCN Manager, and Digitial & Transformation
Lead.
Experience
Essential
- Must have some experience of working in general practice.
- Must have experience of either working in social care or acute community care
- Experience or understanding of co-ordinating an MDT in a health or social care setting
- Excellent organisational skills
- Excellent communication skills
Qualifications
Essential
- GCSE grade A-C in English & Maths
