The Care
Coordinator will support the practice in delivering proactive, coordinated and
well-tracked patient care.
will focus on recall activity, planned care coordination, patient engagement,
NIS, LES, QOF and local contractual work, EMIS searches, trackers, reporting
systems, external service links and safe escalation of concerns.
post-holder will work within a non-clinical role, supporting patients to access
appropriate care and ensuring clinical questions, safeguarding concerns or
risks are escalated to the appropriate clinician or manager.
