  • To support the admin team with Urgent and Non urgent referrals.
  • Involvement in patient tasks and email responses.
  • Management of the patient Home Visit request book.
  • Act as Patient Complaint first point of contact.
  • Work closely with the management team, to prioritise clinical team workload.
  • To make effective use of the Clinical Team appointments book, contacting patients using all IT packages.
  • Processing of patient records.
  • Arranging clinical meetings and providing meeting minutes.
  • Taking ownership of patient information boards and screens.

Utilise population health intelligence to

proactively identify and work with a cohort of patients to deliver

personalised care.

Support patients to utilise decision aids in

preparation for a shared decision-making conversation.

Holistically bring together all of a persons

identified care and support needs, and explore options to meet these

within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person.

Help people to manage their needs through

answering queries, making and managing appointments, and ensuring that

people have good quality written or verbal information to help them make

choices about their care.

Support people to take up training and

employment, and to access appropriate benefits where eligible.

Support people to understand their level of

knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing, including

through the use of the Patient Activation Measure.

Assist people to access self-management

education courses, peer support or interventions that support them in

their health and wellbeing and increase their activation level.

Explore and assist people to access personal

health budgets where appropriate.

Provide coordination and navigation for people

and their carers across health and care services, working closely with

social prescribing link workers, health and wellbeing coaches, and other

primary care professionals.

Support the coordination and delivery of MDTs.

is a two Partner practice, located in a small village in Cheshire. We have two salaried GP's, four Nurses and one GPA. This is supported by

a Practice Manager and Senior Receptionist working with a small administration

and reception team. We form part of the local SMASH PCN group of Surgeries,

working together to focus on local patient care and to deliver local services.

Together we are a friendly team who work

closely and meet on a regular basis to promote continuous learning and

professional development for all.

We are supportive of stretch and challenge, of all team members and roles.

More recently we have been involved with the training of new Nurses, with the Cheshire and Merseyside training hub.

Our patient population is rapidly expanding, due to new housing developments in the local area, and we are currently in the process of extending the surgery to meet these needs.

Qualifications

Essential

  • NVQ Level 3 or equivalent level of knowledge in office procedures
  • Experience of working under own direction
  • Experience in a patient focused environment
  • Evidence of experience in wide range of administrative systems and software programmes
  • Experience of planning and organising complex meetings/agendas
  • Evidence of working with IT systems
  • Evidence of ability to support collation and analysis of data
  • Excellent verbal and written skills
  • Ability to provide and receive complex information
  • Excellent interpersonal skills
  • Demonstrable ability to show kindness and compassion
  • Demonstrate ability to reflect and learn from situations
  • Identifies difficulties as challenges and works with others to identify solutions
  • Demonstrates co-operative team working and awareness of the roles of other professionals
  • Able to work on own initiative, organising and prioritising own workload to set deadlines
  • Ability to adapt and change approach as circumstance dictates

Desirable

  • Experience of supporting service improvement
  • Previous experience in NHS, Primary Care, Local Authority role
  • Experience of using a medical software package e.g. SystemOne and EMIS Web
  • Proven track record of effective use of networking and influencing skills
