Salary: £30,000 - £34,000| Location: South Tyneside | Type: Full-time, Permanent
We are recruiting on behalf of a values-led care organisation for a Care Coordinator who will be responsible for coordinating and supporting the delivery of high-quality, person-centred services for adults with learning disabilities, attending the day centre. The role involves developing support plans, liaising with families and external professionals, coordinating activities and services.
- Maintain care plans, risk assessments, and care records.
- Ensure compliance with CQC, safeguarding, GDPR, and health & safety standards.
- Support audits, quality assurance, and continuous improvement initiatives.
- Liaise with service users, families, professionals, and external agencies.
- Investigate and record incidents, complaints, and safeguarding concerns.
- Provide direct care support when required.
- Experience in care coordination, administration, or support services.
- Knowledge of care planning, safeguarding, and CQC requirements.
- Strong IT skills, including Microsoft Office and digital care systems.
- Organised, detail-focused, and able to manage multiple priorities.
- Full UK Driving Licence and willingness to complete an Enhanced DBS check.
