About the Role We are seeking a caring, organised and motivated Care Coordinators to join our friendly GP practice team. This role combines personalis...
Proactively identify and work with people who would benefit from care coordination, including those with complex needs.
Support patients to develop and review their personalised care and support plans.
Act as a central point of contact, helping patients navigate health, social care and community services.
Liaise closely with GPs, nurses, social prescribers, health and wellbeing coaches and other MDT members.
Assist with followup after hospital care, transitions between services, and ongoing communication.
Provide a welcoming, professional reception service to all patients and visitors.
Book, amend and triage appointments (telephone, facetoface and online).
Process home visit requests, clinical messages and signposting queries.
Handle incoming calls, emails, online requests and post.
Support with test result queries and specimen handling in line with protocols.
Register new and temporary patients and update patient information.
Process repeat prescriptions following practice guidelines.
Scan, index and code clinical documents using appropriate SNOMED codes.
Maintain high quality record keeping and support data collection for audits/service improvement.

Essential: Previous experience in a public facing role and/or admin/reception environment.
Desirable: Experience supporting people with long term conditions or working in health/social care.

Role 1 - 28.5 hours per week as follows:
Monday 8am - 6.30pm
Tuesday 9.30am - 8pm
Wednesday 8am - 6.30pm
Role 2 - 38 hours per week as follows:
Monday 8am - 6.30pm
Wednesday 8am - 6.30pm
Thursday 8am - 6.30pm / 9am - 7.30pm (on alternate weeks)
Friday 8am - 6.30pm
Rate of Pay: £12.71 per hour
Annual Leave: 5.6 weeks annual leave inclusive of bank holidays. Entitlements are pro-rata for part-time employees.
Pension: Option to join NHS Pension scheme.

Helix Medical Centre is based within Herts and Essex Community Hospital in Bishop's Stortford. We pride ourselves on delivering professional medical care in a friendly environment while striving to meet all the challenges of modern General Practice.
We are members of the local Primary Care Network providing extended services as a hub to the local community. We also work with Allied Health professionals together with an expanding complement of ARRs staff including Physicians Assistant, Clinical Pharmacist, Community Paramedic, Community Midwife, Health and Wellbeing Advisor, Care Co-ordinator, Social Prescribers and First contact physio, Dietician etc. There is a wide socio-economic and demographic served within our catchment area providing an interesting and varied case mix.

Experience
Essential
- Candidates must be able to work cohesively as part of a team and be capable of working in a fast paced and at times stressful environment. Positivity and a robust nature are a must!
- You will be required to cover additional hours re staff holidays and sickness sometimes at short notice, which may include a later finish.
- The ideal candidate must be confident, a quick learner, possess excellent communication and organisational skills, have a polite telephone manner along with the ability to deal with sensitive / confidential issues appropriately whilst remaining calm under pressure.
Desirable
- Previous experience working in a medical environment
- Previous experience working in a GP practice
- Able to handle a fast paced environment
- SystmOne Experience
- Access to an NHS Smartcard
