Job description

We are looking for an experienced Registered Nurse or Allied Health Professional to join our Integrated Front Door Service as a Bank Senior Clinician....

The main duties of the role will include undertaking senior clinical assessment and triage of patients with acute, complex or changing needs; supporting timely clinical decision-making; and identifying appropriate community-based alternatives to hospital admission. The post holder will work collaboratively with multidisciplinary and multi-agency colleagues to support urgent community response, virtual ward, rehabilitation, specialist community services and safe discharge pathways. They will contribute to board rounds, MDT discussions and care planning, provide clinical advice and support to colleagues, maintain accurate clinical records and promote person-centred, evidence-based care that helps reduce delays, unnecessary admissions and length of stay.

Training and Qualifications

Essential

  • Current registration with NMC or HCPC without restrictions on practice.
  • Degree or equivalent relevant experience, supported by additional specialist training, short courses or a professional portfolio.
  • Evidence of continuing professional development relevant to urgent, emergency or community care.

Experience

Essential

  • Significant relevant post registration experience.
  • Recent experience in acute, emergency, urgent community response, admission avoidance or community services.
  • Experience of undertaking clinical assessment and contributing to safe clinical decision-making.
  • Experience of supporting patient flow, discharge planning or admission avoidance pathways.
  • Experience of supervising, supporting or providing leadership to junior staff or students.

Knowledge and Skills

Essential

  • Sound clinical judgement and decision-making skills when assessing patients with acute, complex or changing needs, including analysing information from a range of sources and comparing appropriate care pathway options.
  • Health assessment and physical examination skills in clinical practice, or willingness to work towards.
  • Knowledge of current community services, urgent care pathways and admission avoidance services.
  • Ability to triage referrals, prioritise workload, coordinate activity across multiple pathways and respond effectively to competing clinical priorities.
