Bank Physiotherapist

• Deliver a wide range of specialist physiotherapy assessment and treatment as part of the Unplanned Care Service.
• Work with patients with a range of complex needs, supporting timely discharge from acute care and preventing hospital admission.
• Provide a comprehensive physiotherapy service to residents of West Hertfordshire referred into the service.
• Work primarily across the Discharge to Assess and Rapid Response pathways, with the ability to work across other pathways according to service requirements.
• Assess and manage an autonomous caseload, developing and implementing appropriate treatment and rehabilitation plans.
• Support patients to maximise their independence and achieve their rehabilitation goals within their own home or community environment.
• Contribute to effective discharge planning, working collaboratively with patients, families, carers and the wider multidisciplinary team.
• Undertake equipment assessment, prescription and provision where clinically appropriate.
• Work confidently as a lone practitioner within community settings, recognising and managing risk appropriately.
• Maintain accurate and timely clinical documentation in accordance with professional standards and organisational requirements.
• Work within professional standards, clinical guidelines and best practice to ensure safe and effective patient care.
• Contribute actively to the wider clinical team and multidisciplinary decision-making.

Essential

• Diploma/degree in Physiotherapy or Occupational therapy
• Current HCPC registration
• Evidence of up-to-date CPD portfolio
• Involvement in Appraisal Scheme
• Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation
• Member of Professional Body

• Member of a Special Interest Group relevant to this post

Essential

• Experience post registration as a Band 5 Physiotherapist or Occupational therapist
• Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory.

• Experience working with voluntary organisations.
• Experience in service development projects.

Essential

• Skills in the assessment and treatment of adults with complex multi-pathologies
• Knowledge and understanding of complex medical conditions including multipathologies and falls
• Knowledge of role of Allied Health Professionals and interdisciplinary team working
• Ability to apply core Physiotherapy/Occupational therapy skills across a range of clinical conditions within a client centered, goal setting
• Knowledge of outcome measures and evidence-based practice
• Ability to problem solve, prioritise and demonstrate clinical reasoning
• Basic IT skills - Microsoft Word, Power point, Excel
• Teaching of junior staff / students, peers in Inservice training setting
• Knowledge and understanding of the role of Social Services and voluntary agencies
