We are seeking an experienced and motivated Transition Lead Nurse (Band 7) to lead a high-profile programme focused on improving the experience and outcomes of young people transitioning from children's to adult health and care services. This is an exciting opportunity to drive transformational change across a complex system, working in partnership with health, social care, education, voluntary sector and independent organisations. As the Transition Lead Nurse, you will be responsible for developing, implementing and embedding integrated transition pathways, policies, Standard Operating Procedures (SOPs), and best practice guidance that promote consistent, safe and person-centred care. The post holder will work collaboratively with a wide range of stakeholders to reduce variation in practice, strengthen partnership working, and ensure young people receive coordinated support as they move between services. You will provide leadership and expert guidance to professionals across organisations, helping to establish clear frameworks that support effective transition planning and improve long-term outcomes. The successful candidate will have significant clinical experience, excellent project management and stakeholder engagement skills, and a passion for improving services for young people. Essential
- Registered Nurse (NMC Registered). Degree in Nursing or equivalent professional qualification. Evidence of continuing professional development.
