Job Title: Physiotherapist - 91839 Grade: Band 6 Trust: Berkshire Healthcare NHS Foundation Trust Location: Wokingham Community Hospital, RG41 2RE War...
- Be able to assess and provide high standards of initial assessment input to patients in the community.
- Be able to liaise with MDT members in treatment and management care plans and working closely with therapist.
- Create, evaluate all care plans implemented and should have a sound knowledge in discharge planning and ability to make further referrals in to the community as required.
- Devise Physiotherapy treatment plans and goals in collaboration with the patient, relatives, carers and the interdisciplinary team.
- Be highly skilled in using and reporting on standardised and non-standardised Physiotherapy assessment to diagnose and analyse highly complex patient needs in the context of activities of daily living.

Qualifications
Essential: - Physiotherapy Qualification and experience working with the elderly mandatory.; - HCPC registration mandatory.
Desirable: - RiO training desirable.
