Band 6 Mental Health Practitioner – Non-ARRS

As a Band 6 practitioner, you will undertake autonomous clinical work while remaining closely connected to the wider multidisciplinary team.

The role will include

-

Undertaking holistic mental health assessments, including risk, strengths, functioning, physical health and social circumstances.

Developing collaborative formulations, care plans and safety plans.

Providing brief, evidence-informed interventions and agreed follow-up.

-

Holding and reviewing an appropriate caseload in line with the needs of the service.

Liaising with GP surgeries, primary care mental health practitioners, CMHTs, social care and voluntary and community organisations.

Supporting people to access the clinical, practical and community resources best placed to meet their needs.

Contributing to multidisciplinary discussions, allocation processes and decisions about onward referral or discharge.

Working across North Birmingham and from other Trust, primary care or community locations when required, subject to service need.

Maintaining clear clinical records and communicating plans effectively across services.

Contributing to clinical governance, service development and the learning of students and colleagues.

Contacts may be undertaken remotely or from Trust, primary care and community settings, according to clinical and service need.
