North and South GP Integrated Mental Heath Services (IMHS) are seeking an enthusiastic Psychiatric Nurse to join the team. Please note the role is par...
- upporting the Mental Health Practitioner and Clinical Lead with reviewing and allocating new referrals to team members
- Assessing the mental health of patients in initial appointments and subsequent follow ups
- Offering a holistic assessment and treatment plan in collaboration with the patients, and as needed, carers and other professionals involved in their care
- Work with partnership professionals such as GPs, social care, housing and community connectors to create plans to address the needs of your patients. This includes initiating referrals (e.g. to Adult Social Care, to the VSCE employment support service etc.)
- Offer brief interventions to the patients you see, within the 4 appointments, as per the short-term model of the team. These interventions may include behavioural activation, coping skills for anxiety and low mood and emotional regulation skills. These skills are informed by a CBT, DBT or solution focused therapy approach
- To work within a model of recovery, early intervention and solution focussed to empower patients to lead their own recovery and avoid dependence on mental health services alone
- To partake in the weekly Multi-Disciplinary Team (MDT) meeting and support colleagues manage complex cases, as well as present your own cases that you need support with
- As needed, present complex cases to the wider MDT, through Pathways Forum, to seek support from colleagues within the Complex Needs team, Talking Therapies and the psychology team.
