Your role will provide support patients with conditions like COPD, Asthma, Bronchiectasis and Interstitial Lung Diseases (ILD) within the community and provide a rapid response service to prevent unnecessary hospital admissions or early supportive discharge from hospital for patients with acute exacerbations of their chronic respiratory disease.

The post holders will work as part of the ACERS Acute Respiratory Infections Virtual Ward, using their specialist skills to improve patient outcomes within the home environment. The role will include working with local GP practices to provide education and provide a case management approach in a supported environment.

  • NMC or HCPC registration
  • Nursing qualification or Physiotherapy degree

  • Evidence of relevant post graduate training
