Advanced Nurse Practitioner

The Advanced Nurse Practitioner (ANP) will act

autonomously within their professional scope of practice providing care for

patients face to face, telephone consultations and home visits.

The post holder will operate within their own

sphere of competence and agreed acceptable limits of practice to provide expert

professional clinical advice to patients, carers and colleagues, ensuring

clinical safety and excellence.

The Advanced Nurse Practitioner will need to

prioritise and triage the needs of patients accordingly, making any necessary

referrals for investigations in the appropriate manner.

The post holder will use skills, knowledge and

competencies as a qualified Advanced Nurse Practitioner in order to be

responsible and accountable for managing patient caseloads for treatments,

referral, admission and discharging patients as appropriate.

The practice uses Vision, Docman mail

system and askmyGP. Experience in these systems would be advantageous and good

IT skills are essential.

Responsible for the

assessing, diagnosing, prescribing and treating of patients who are

registered within the GP

practice (as well as for non registered patients from other practices

whom the practice has

responsibility for through any of its enhanced services schemes).

The post holder will participate fully in the

delivery of a high quality, co-ordinated health

care system delivery for

patients catered for by the practice. There will also be

responsibility for

training, supervision of and mentoring of the Nursing Team including

Woodbank Surgery is a GP practice with a

list size of approximately 4,700. We have 2 salaried GPs, 1 HCA, 1 Practice

Nurse, 2 Clinical Pharmacists (job-share) and a non-clinical team of 8,

including an Office Manager and Practice Manager. The practice was rated Good

in our last CQC inspection; we have a high QoF achievement and strive to

provide the best quality care possible to all of our patients.

Qualifications

Essential: - RGN 1; - MSc in Clinical Nursing or Nurse Practitioner Degree Level.; - Independent Prescriber

Desirable: - Mentorship & Assessing at Level 111 (or equivalent ENB 998); - Other Post Registration Long Term Conditions Qualifications

Experience

Desirable: - Practice Nurse Experience
