- demonstrate a high level of expertise within the specialist service providing advice, education & support to staff, patients, families and carers.
- be practising autonomously as an ACP within the designated speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis & treatment within an agreed scope of practice throughout the directorate.
- support new ways of working that emphasises a more efficient & patient-focused service, & will ensure the safe treatment, referral & discharge of patients with undifferentiated & undiagnosed presentations in their area.
- have responsibility for leading the on-going development of clinical practice & standards of care within the service, including the development of policies, procedures, protocols & guidelines in collaboration with multidisciplinary colleagues.

As an advanced nurse practitioner, you will play an important role in providing high-quality, patient-centred care to our practice population. The role will include assessing, diagnosing and managing patients presenting with a wide range of health conditions, working both independently and as part of our wider clinical team. You will have the opportunity to use and further develop your clinical skills within a supportive primary care environment. We are looking for someone who is::
Essential
- An experienced and appropriately qualified advanced nurse practitioner
- Confident in assessing, diagnosing and managing patients with a range of acute and long-term conditions
- A strong communicator with excellent interpersonal skills
- Patient-focused, compassionate and committed to delivering high-quality care
- Able to work independently while also being a valued member of a multidisciplinary team
- Enthusiastic, adaptable and keen to contribute to the continued development of the practice
