ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate and refer to other specialists if necessary.
Develop,
implement and embed health promotion and wellbeing programmes
Manage
patients presenting with a range of acute and chronic medical conditions,
providing subject matter expert advice
Implement
and evaluate individual specialised treatment plans for chronic disease
patients
Identify,
manage and support patients at risk of developing long-term conditions,
preventing adverse effects to the patients health
Provide
advanced, specialist nursing care t patients are required in accordance with
clinical based evidence, NICE and NSF
Provide
wound care (ulcer/doppler, etc)
Provide
specialist clinics such as contraception etc
Undertake
the collection of pathological specimens
Provide
travel medicine services
Request
pathology services as necessary
Process
and interpret pathology and other test results as required
Provide
chronic disease clinics, delivering patient care as necessary, referring
patients to secondary/specialist care as required
Maintain
accurate clinical records in conjunction with extant legislation
Ensure
SNOMED CT codes are used effectively
Maintain
chronic disease registers
Develop,
implement and maintain well woman, well man and travel clinics
Take part
in implementation of an effective immunisation programme
Prioritise
health issues and intervene where necessary
Support
the team in dealing with clinical emergencies
Recognise,
assess and refer patients presenting with mental health needs
Implement
vaccination programmes for adults and children
Be an extended
and supplementary prescriber, adhering to extant guidance
Support
patients in the use of their prescribed medicines or over the counter medicines
(within own scope of practice), reviewing annually as required
Contribute
to practice targets (QOF etc), complying with both local and regional guidance
Liaise
with the external services/agencies to ensure the patient is supported
appropriately (vulnerable patients etc)
Delegate
clinical responsibilities where appropriate (ensuring safe practice and that
the task is within the scope of practice of the individual)
Support
the clinical team with all safeguarding matters in accordance with local and
national policies
Understand
practice and local policies for substance abuse and addictive behaviour,
referring patients appropriately
Deliver
opportunistic health promotion where appropriate
Market Cross Surgery is under new management.
You will be joining an established clinical team working alongside both our GP's and Nursing team to provide care to our patient population.
We are also a dispensing practice.
Qualifications
Essential
- Masters degree in advanced clinical Practice or Post-graduate diploma/degree for Advanced Practice
- Independent Nurse Prescriber
Experience
Essential
- Experience of working within Primary Care
- Experience of chronic disease management
- Wound care/removal of sutures and staples
- ECGs
- Venepuncture
- New patient medicals
- Requesting pathology tests and processing results
- Understanding requirement of PGDs and associated policies
- Womens health (cytology, contraception, etc)
- Immunisations (routine, childhood and travel)
- Chronic disease management (Diabetes, Asthma, COPD, etc)
- Travel Medicine
- Spirometry
- Experience in infection prevention and control measures
Desirable
- Experience in a GP practice
- Experience with SystmOne
- Experience leading a nursing team
- Experience managing elderly and housebound patients
- Clear, polite telephone manner
- Competent in the use of Office and Outlook
- Ability to follow clinical policy and procedure
- Effective time management
