Advanced Nurse Practitioner

We're looking for a full-time enthusiastic and experienced

Advanced Nurse Practitioner to join our friendly and supportive team at Betts

Avenue Medical Group. This is an opportunity to work in a well-established

clinical environment where you will be delivering autonomous, patient-centred

care across a wide range of acute, urgent and long-term conditions. You'll

assess, diagnose and manage patients, providing safe, evidence-based care while

working closely with GPs and other healthcare professionals to ensure the best

possible outcomes.

initiatives and help shape the future delivery of primary care within our

Betts Avenue is a long-established GP Practice operating

over two sites (Kenton and Benwell) in Newcastle upon Tyne. We are a team of 2

GP Partners supported by 3 salaried GPs, 2 long-term Locum GPs, a Lead practice

Nurse/Advanced Nurse Practitioner, 1 Advanced Care Practitioner, 2 Practice

Nurses, 1 HCA as well as an experienced managerial and admin team. We deliver a

range of GMS services to a population of approximately 10,500 patients. We also

have support from Care-Coordinators, Workflow Team, Pharmacy Hub, PCN

pharmacists and Technicians, House Visiting team, our 2 own Practice Clinical

Pharmacists, a Physiotherapist, 2 Mental Health co-ordinators for adults and a

team of Social Link Workers. We use SystmOne, Accurx and AnalyzeRx. We are also

delivering weekly Ward Rounds in two large local Care Homes. We currently teach Year 5 Medical Students

and Nursing Students. Our practice is a member of the Inner West Primary Care

Network, LMC, NGPS and North East North Cumbria ICB.

Duties and responsibilities specific to this post

Independently assess, examine, diagnose and

manage patients presenting with undifferentiated and undiagnosed conditions.

Develop, implement and evaluate evidence-based

treatment and management plans.

Prescribe safely and effectively as an

Independent Prescriber, in accordance with national guidance and local

policies.

Request, interpret and act upon diagnostic

investigations, including pathology and imaging results.

Recognise and appropriately manage medical

emergencies, escalating or referring patients when required.

Refer patients to secondary care, community

services and other healthcare professionals using appropriate referral

pathways.

Promote health, wellbeing and disease prevention

through patient education and lifestyle advice.

Manage patients with both Acute presentations

and Long-term conditions, ensuring continuity of care where appropriate.

Maintain accurate, timely and comprehensive

clinical records using SystmOne.

Participate in clinical governance activities,

audits, quality improvement projects and significant event reviews.

Contribute to the development of practice

protocols, policies and clinical pathways.

Support the education and development of

colleagues, students and trainees where appropriate.

Work collaboratively with the wider

multidisciplinary team to ensure high standards of patient care and service

delivery.

Comply with all professional, organisational and

statutory requirements, including safeguarding, infection prevention,

information governance and health and safety policies.
