Responsibilities of the role will include

  • Acting as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
  • Working as an autonomous clinical practitioner/nurse, providing high quality and timely patient centred care.
  • Independently prescribe, working within the current guidelines, reviewing, and monitoring patient medications in accordance with the formulary.
  • Delivering face to face/telephone/e-consult/video consultations as well as home visits.
  • Directing care appropriately and managing complete episodes of care independently.
  • Undertaking consultations with patients of all ages including children as an autonomous practitioner and using own clinical judgement, diagnose, treat, refer and/or discharge patients.
  • Working within and across the MDT teams and signpost/draw upon expertise of all members to support and meet the patients' needs and best interests.
  • Recording clear and contemporaneous consultation notes to agreed standards, ensuring accurate read coding.
  • Participating in, and contribute to, all clinical targets (QOF, QIS, ES etc).
  • Recommending, instigating, and explaining appropriate invasive and non-invasive diagnostic tests, investigations and interpret findings and reports.

East: MK PCN covers a total population of around 56,000 patients, across its 4 practices in Milton Keynes: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and the Grove Surgery.

Essential qualifications, skills and experience:

Essential

  • Registered nurse with the Nursing and Midwifery Council (NMC) or registered with HCPC.
  • Masters degree from the Centre for Advancing Practice accredited MSc advanced practice programme or have completed the centres e-Portfolio (supported) route; both pathways enable eligibility for an Advanced digital badge issued by the centre, demonstrating recognised educational and experiential preparation in advanced practice.
  • A qualified independent prescriber, on the relevant register.
  • Meet NMC revalidation requirements, in accordance with the NMC revalidation booklet.
  • Meets the standards for registered Advanced Practitioner working at advanced level.
  • Ability to record accurate clinical notes.
  • Ability to work within own scope of practice and understand when to refer to a GP.
  • Experience of working in a management role and evidence of leadership skills.
  • Experience of working in a wide range of primary care settings to include clinical triage experience.
  • Ability to problem solve and to process information accurately and effectively, interpreting data as required.
  • Knowledge of health promotion strategies.
  • Understand the requirement for PGDs and associated policy.
  • Polite, confident, flexible and cooperative.
  • Motivated and a forward-thinker.
  • High levels of integrity and loyalty.
  • Sensitive and empathetic in distressing situations.
  • Ability to work under pressure in stressful situations.
  • UK driving licence, with exclusive access to a vehicle.
