The post holder will be an integral part of the clinical management team, demonstrating advanced autonomous clinical practice skills across MWL, the post holder will act as the lead for the Dementia and Delirium team across MWL
Acting within their professional boundaries, they will demonstrate safe, clinical decision-making skills and will manage their own caseload including the treatment and care of patients from initial history taking, assessment, diagnosis, treatment, and evaluation of care for people presenting with Dementia and / or Delirium.
- Expert Clinical Practice - Conduct comprehensive advanced assessments for older patients presenting with cognitive impairment, formulate evidence-based management plans for complex dementia patients.
- Leadership and Innovation - Act as a clinical role model, driving quality improvement initiatives and implementing best practice guidelines across the service & provide line management for the D&D team across MWL.
- Education & Mentorship - Provide Specialist training, education and mentorship to nursing staff, allied health professionals and medical teams to enhance organisation wide understanding of cognitive impairment.
- Collaborative Care - Partner closely with families, carers psychiatric liaison teams and community services to ensure seamless, holistic patient care.

We also provide urgent care at our Urgent Treatment Centre located in St Helens town centre.
Specialist Regional Services
We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.

Qualifications
Essential
- Registered senior clinician with current NMC / HPC registration
- MSc Advanced Practice / actively working towards
- Independent / supplementary nursing prescribing - V300
- Clinical Assessment & Diagnostics Qualification
- Portfolio of evidence demonstrating up to date achievement of required competencies for advanced practitioner practice
- Evidence of on-going professional study and personal development
- ALS provider or willing to work towards
Desirable
- Leadership qualification
- NMC Specialist Community Practitioner Qualification
- Teaching/assessing/NMC Mentorship Qualification

Knowledge and Experience

- Ability to formulate differential diagnosis, initiate appropriate treatment and evaluate effectiveness.
- Experience and evidence of working collaboratively in a multidisciplinary environment.
- Evidence of effective leadership, practice development, expertise and innovation in clinical practice
- Experience of clinical practice benchmarking
- Highly motivated and experienced practitioner

- Experience of nurse triage including telephone triage
