- To support the Team Leader by carrying out specific health care assessments and/or interventions
- To carry out initial assessment and triage of patients with short term health care needs by telephone, at clinical settings or in patient's homes under the supervision of a Team Leader
- To provide continuity of care for children on caseload and maintain contemporaneous records accordingly.
- To discharge patients according to pathways of care, under supervision of Team Leader.
- May carry out initial home assessments with guidance from the Team Leader to plan, implement and evaluate programmes of care.
- To work in partnership with families and the multi-disciplinary team to provide care in appropriate settings to maximise life opportunities for the child or young person.
- Report any changes in the child's condition to the Team Leader and medical colleagues and ensure all records are maintained in accordance with Trust policies.
- Participate in the provision, storage and administration of medicines including intravenous therapy in accordance with Service/Trust guidelines and policies and in partnership with pharmacy colleagues.
- Participate in the collection of data as required for evaluation of the service and take part in audit as required by the Trust.
- Undertake specific clinical tasks and skills for which the nurse has received training and is deemed competent in all clinical settings.

Qualifications

- RN Child
- Evidence of ongoing development
- Teaching/Mentorship qualification or willingness to undertake.
Experience
Essential
- Experience of nursing children with acute/short term health needs.
- Experience of interagency working.
- Experience of effective working collaboratively with professionals from other disciplines.
Desirable
- Experience of nursing children with long term conditions, complex needs and disabilities.
- Experience of planning and delivering training.
