Job description
Job summary We are looking to recruit a Young Persons Care Coordinator to work as part of our Social Prescribing Team. Social prescribing connects and signposts people to practical and emotional support such as peer support groups, activities, housing, finances and lots more. The Young Persons Care Coordinator will support our 5-25-year-old patients. The working pattern will be: Monday 9am-5pm, Tuesday 9am-5pm, Wednesday 9am-4:30pm, Thursday 10am-6pm and Friday 9am to 5pm (total of 37 hours). W…
