Job overview As part of a team of Clinical Pharmacists and technicians you will be responsible for providing safe and effective medicines optimisation services to patients referred into the Integrated Pharmacy Team (IPT) and agreed community hospital sites. In addition, you will provide pharmaceutical advice to a range of specialist services.
