Job description
Summary This position will support the Emergency Department. The Palliative Care Nurse Navigator is a specialized registered nurse who guides patients and their families through serious or life-limiting illnesses. They coordinate care, triage referrals, facilitate goals-of-care discussions, provide education regarding disease processes, and help connect patients to community resources to improve their quality of life.
