Are you a Registered Nurse looking for a new challenge? Do you have the passion and drive to make a real difference? If so, we'd love to hear from you.

An exciting opportunity has arisen for a Nurse Case Manager to join Routes Healthcare, where you'll play a pivotal role in supporting and overseeing complex care packages — maintaining the highest standards of clinical excellence and facilitating truly person-centred care.

A little about us

At Routes, we're an incredible team of people from all walks of life, brought together by one thing: we all care about making a difference. Not just for our clients, but for our workers too.

For the last 15 years, we've been delivering high standards of care that put people's wants and needs at the heart of everything we do — helping people live the way they choose, in the comfort of their own home.

If that sounds like a team you'd like to be part of, we're already on to a winner. But let us sweeten the deal a little more.

We want to help you flourish — Routes gives you the tools so you can focus on what truly matters: delivering excellent care to your community.

What does that look like in practice?

  • Centralised support teams keeping things running smoothly in the background — HR, Payroll, Quality and Marketing to name a few
  • Support to enhance your learning, NMC revalidation and regular clinical supervision, backed by a network of experienced clinicians
  • Our in-house recruitment team ensuring you have the staff you need, with zero agency use
  • Routes Academy — keeping the team highly trained and giving both you and our healthcare workers real opportunities to progress and develop
  • Digital Care Plans for a more efficient, modern operation
  • Our own friendly IT team on hand whenever you need them

  • Wellbeing and financial support through our Employee Assistance Programme (EAP)
  • 25 days holiday plus bank holidays — and an extra day off for your birthday
  • Buy and sell holiday scheme
  • Pension scheme
  • A competitive salary plus an attractive bonus package based on service performance
  • A fun, friendly and genuinely supportive place to work

What your day-to-day will look like

  • Managing complex care packages, both clinically and holistically
  • Developing, reviewing and implementing care plans and risk assessments
  • Managing the clinical training needs of the care team, maintaining an appropriate skill mix to ensure safe and effective practice
  • Engaging in networking, attending MDT meetings and working with clinical commissioning groups
  • Conducting new and ongoing competency assessments with care staff
  • Carrying out care package reviews
  • Auditing client notes and medication records

What we're looking for

  • RN qualification (Adults) with a valid NMC pin
  • Experience in a high dependency or critical care setting (advantageous)
  • Experience in tracheostomy and ventilation management in a community setting (desirable)
  • Computer literate with working knowledge of Word, Excel and Outlook
  • A good understanding of CQC regulations
  • Excellent communication skills, both written and verbal
  • Someone passionate about putting people first
  • Flexible, autonomous and well-organised
  • Access to a car for work purposes

This could be the start of something really special — for you and for the people in our community whose lives you'll help to change. We'd love to hear from you.
