Job description
Completing holistic clinical assessments and developing personalised care plans
Managing and reviewing complex care packages within the community
Providing clinical leadership, supervision and support to care teams
Working closely with clients, families and multidisciplinary professionals
Ensuring high standards of clinical governance, quality and compliance
Identifying and implementing improvements to care delivery and outcomesAbout You To be considered, you'll need:
Recent hands-on experience with tracheostomy and ventilator care
Experience supporting, mentoring or supervising care staff
Strong clinical assessment and care planning skills
Confidence working autonomously within the community
Full UK driving licenceWhat's on Offer?
Salary up to £40,000 DOE
Monday-Friday working pattern
Hybrid working – 2 days office-based / 3 days client-based or home-based
No nights or regular weekend shifts
