Job description

Holistic Recruiters Ltd is seeking a compassionate, resilient and recovery-focused Recovery Practitioner to join the team at Crowlin Recovery House, a 24-hour NHS-commissioned residential service providing a safe and supportive alternative to hospital admission for adults experiencing mental health crises.

Working as part of a multidisciplinary team, you will provide high-quality, person-centred and trauma-informed support to guests during periods of emotional distress or mental health crisis.

You will help guests stabilise, develop coping strategies, maintain their wellbeing and independence, and work towards their individual recovery goals and safe return home.

Provide person-centred emotional, practical and recovery-focused support.
Build positive, professional and therapeutic relationships with guests.
Support guests to identify their strengths, needs, goals and recovery priorities.
Encourage the development and use of healthy coping strategies.
Promote independence, confidence, wellbeing and self-management.
Support guests with daily routines and independent living skills.
Develop, implement and review Recovery, Wellness, Safety and Return Home Plans.
Facilitate and participate in recovery-focused activities, including wellbeing, mindfulness, life skills and confidence-building sessions.

Support guests during periods of mental health crisis and emotional distress.
Complete dynamic and ongoing risk assessments.
Recognise changes in presentation, behaviour or mental wellbeing and respond appropriately.
Follow agreed risk management and safety plans.
Respond professionally to incidents and emergencies.
Identify and report safeguarding concerns promptly.
Maintain professional boundaries and promote a safe, respectful and therapeutic environment.

Work collaboratively with Crisis Resolution and Home Treatment Teams (CRHTTs), NHS professionals and other partner agencies.
Contribute to effective communication between the Recovery House, guests, families and relevant professionals.
Participate in handovers and share relevant information appropriately.
Support admissions, orientation and the safe settling-in of new guests.
Contribute to effective discharge planning and guests' transition back home or to appropriate onward support.

Maintain accurate, timely and confidential records.
Complete daily progress notes, support documentation, care/recovery plans and risk assessments.
Follow organisational policies, procedures and professional standards.
Maintain confidentiality and comply with information governance requirements.
Report incidents, concerns and changes in risk appropriately.
Attend team meetings, supervision, training and other required sessions.
Maintain mandatory training and professional development requirements.

Level 3 Diploma in Health & Social Care, or willingness to work towards the qualification.
Previous experience supporting adults with mental health needs, vulnerable adults or individuals experiencing emotional distress.
Previous experience within health or social care. Understanding of mental health recovery and person-centred support.
Understanding of safeguarding, confidentiality and professional boundaries.
Good written and verbal communication skills.
Ability to maintain accurate and professional records.
Ability to remain calm, compassionate and professional when working in challenging or crisis situations.
Ability to work effectively as part of a team.
Flexible approach to working shifts, including evenings, waking nights, weekends and bank holidays.
