Job description
We are looking for caring and proactive individuals to provide personal care to 2 service users in their own home. Staff will liaise with the district nurses and health professionals to carry out all identified social care tasks and healthcare takes assigned by them, whilst ensuring and outcome-focused service which is responsive to individuals changing needs.
