Job description
Job title: Central Digital Pharmacist
Location: Hubs and IUC
Salary: Up to £55,000 depending on experience
Hours: 37.5 (Monday to Friday)

As a valued member of the Operose Health team, your key duties will include:
- As a member of the practice team, you will work closely with our clinicians to help and improve patient care and make cost effective proscribing decisions.
- You will run daily telephone triage clinics and call in patients for assessment and management where clinically indicated, as well as review and triage pathology results, contacting patients. Provide practice-based review clinical to those with LYCs or complex health needs.
- Review and action incoming hospital correspondence, particularly with regards to medicine reconciliation or initiation, liaising with local pharmacies to improve patient outcomes.
- Lead practice repeat prescription service and deal with queries from reception staff and patients. Also participate in home visits and immunisations initiatives.
- GPhC registered.
- Excellent communication skills both verbally and written as well as the ability to work well as a part of a team and willingness to develop clinically.
- A can-do attitude with a real desire to improve patient care.
- Annual salary of up to £55,000 depending on qualifications and experience.
- 27 days annual leave plus bank holidays pro rata.
- Support to gain independent prescribing qualification.
- 1 weeks paid CPD/study leave pro rata per year as well as in house CPD training sessions tailored to meet your needs.
