We are seeking dedicated Community Registered Nurse Band 6 to join our Swindon Integrated Neighbourhood Team. This is an exciting opportunity to become part of a supportive and forward-thinking service that places patients at the heart of everything we do. Working within the community, you will provide person-centred care to patients in their own homes and residential settings, supporting them to manage their health needs while helping to prevent avoidable hospital admissions. You'll work closely with a multidisciplinary team including GPs, Therapists, Clinical Leads, Care Coordinators, and the Home First Team to deliver exceptional care and positive outcomes for patients. We are looking for experienced and compassionate nurses with a background in NHS community or acute care, excellent clinical skills, and the ability to build strong relationships with patients and their families.
