As a Community Nurse you will be part of our valued team at HCRG Care Group, receiving access to exclusive rewards and benefits. You will deliver compassionate, person-centred nursing care, assessing, planning, implementing, and evaluating care interventions that meet the individual needs of patients and their carers. Manage a diverse caseload, ensuring care is coordinated, responsive, and focused on achieving positive outcomes and promoting independence. Conduct holistic assessments using appropriate clinical tools, identifying risks, safeguarding concerns, and changes in patient condition, taking timely action and escalating where required. Undertake a range of clinical procedures within scope of practice. Provide health promotion, education, and advice to patients and carers, empowering them to make informed decisions.

Qualifications

Essential

  • Registered Nurse (Level 1) with current NMC registration and a commitment to practising in line with the NMC Code of Conduct. Significant post-registration nursing experience, ideally within community, district nursing, or related healthcare settings. Proven ability to lead, support, and motivate colleagues while contributing positively to team performance. Experience managing and coordinating complex caseloads, including patients with long-term conditions, palliative care needs, and multiple co-morbidities. Full UK driving licence and access to a vehicle to meet the travel requirements of the role.
