Job summary North & East Integrated Transfer of Care Hub (ITOCH) Team are seeking to recruit a clinical prioritisers to coordinate the local demand and capacity, and to ensure that individuals needs are triaged, prioritised and addressed through the most appropriate resource from the multi-disciplinary and cross organisational team.
