Job description
Job summary We are employed by Community but based on the acute hospital site at NDDH. Our remit is to assess and procure onward care for patients being discharged from hospital on discharge pathways 2 and 3, however, on occasions we may also support with pathway 1. The postholder will be expected to complete comprehensive, holistic assessments, provide expert support to ward staff and resolve discharge delays, working closely with other relevant acute and community Health and Social Care teams…
