Band 6 Mental Health Nurse or Allied Health Professional

• Comprehensive assessment of new referrals in crisis, intervention planning and care plan formulations.
• Care planning for patients who are accessing the on site beds.
• Service coordination duties include, understanding the roles of the team that day, which patients will be seen and identifying any gaps where services could be supported. Attending morning meetings/ referral meetings and Weekley MDT's
• Taking referrals from other services where the person is identified to be registered with a Heeley PCN GP.
• Carrying out initial HTT visit to set up plan of care and interventions for the team
• Link to other clinical areas such as inpatient and CMHT's
• Delivering supervision to more junior members of staff
• Contribute to service development

• Maintain NMC registration and adhere to professional guidelines and code of conduct
• Work within the framework of relevant policy and legislation both local and national

• Triage new referrals to the service
• Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action
• To assess, evaluate and manage clinical risk
• To formulate a plan of care based on identified need
• Deliver evidence based crisis interventions as appropriate
• Review effectiveness of care plan at regular intervals
• To work collaboratively with service users, carers and partner agencies
• To coordinate the clinic work of a day by day basis
• To maintain computer held care records in an accurate and timely fashion
• Undertake all work in a culturally sensitive manner
• Act in accordance with relevant policy and legislation.
• Develop and support effective working relationships with inpatient, community and voluntary organisations.
• Ensure timely and supported step down from acute inpatient setting and provide preventative intervention to avoid unnecessary acute admission.
• Support admissions for service users within the Heeley + PCN to access the onsite hospitality beds to avoid further escalation of crisis.

• Regularly review own clinical work with supervisor to promote efficacy and contribute to the overall management of the HTT & NMHC
• Deliver supervision and facilitate annual PDR with more junior members of staff
• Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution

• Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain sign off status
• Participate in training relevant to role, including training needs for the team.
• Promote insight and understanding of mental health issues to service users, carers, and other agencies
• Contribute to audit, research and evaluation as appropriate

• Contribute to service development and review of development of the NMHC in line with national development of the pilots.

• hold a UK driving license

• Band 6 currently or Band 5 for over 2 years.
• community working experiance
• ability to assess mental health in both a scheduled and crisis capacity
• knowledge and experience of intervention used in crisis services
• experience in devising and implementing care plans.

Desirable

• knowledge of the UK triage rating scale
• experience of triaging
• experience in crisis services
• knowledge of the Neighbourhood mental health center
• experience working across varying sectors of mental health, including primary, secondary and voluntary care sector
