You will work closely with patients, families and carers to deliver personalised care that supports recovery, rehabilitation and independence. Working as part of a multidisciplinary team, you will contribute to the assessment, planning, implementation and evaluation of patient care. You will maintain high professional standards, promote evidence-based practice and ensure patients receive the highest quality care possible. Key responsibilities include:

Previous experience within a ward, community hospital, rehabilitation or intermediate care settingExperience supporting patients with complex health needsMentorship or practice assessor qualificationCommitment to ongoing professional development
