Job description
Completing clinical assessments and care plans
Visiting clients in their own homes
Providing clinical leadership to care teams
Supporting and mentoring nurses and support workers
Working with families and wider healthcare professionals
Ensuring high standards of clinical governance and careWhat you'll need: Registered General Nurse with active NMC registration
Adult complex care experience
Recent tracheostomy and ventilator care experience
Experience supervising or supporting care staff
Strong clinical assessment skills
Full UK driving licencePrevious Case Management experience isn't essential. What's in it for you? Up to £40,000 DOE
