Job summary This is an exciting new post within our Primary Care Network, focused on supporting patients to remain well and independent in their own homes. The Admission Prevention Nurse will provide a proactive, preventative and responsive nursing service for patients at greatest risk of deterioration, urgent care use and unplanned hospital admission.
